You may print as many copies of the form as you need. You should fill out one copy of the registration and contact information for your whole family (please include the names and ages of each camper). You need to fill out a copy of the health care information for each person. Enclose the entire amount owing with your registration and health forms, and mail to Camp Kee-Mo-Kee at the address indicated on the form.
