These pages are available in Adobe Acrobat or Word format. Click on the link below to open the page with the Acrobat Reader (which is probably already installed in your browser). You may print as many copies of the form as you need. You should fill out both pages of the form for each week for which you register. Enclose a $50 deposit with your registration and health form for each week registered.
